5 mg is a 'decent' dose of methadone for RLS. An overview of 'normal' or recommended doses is in the seminal paper on opioids for RLS by the IRLSSG (Internat RLS study group): mayoclinicproceedings.org/a....
Having said that, the situation of the NZ poster is not a standard beginners one as (s)he has already been on a not-low dose of Targinact (slow release oxycodon with naloxon). The poster also doesn't say whether (s)he has completely stopped the other meds when (s)he started the methadone. Maybe in this transfer phase a somewhat higher dose of methadone is necessary. But my adagium is always to try and find your personal minimum effective dose. And never to experiment (too much) without involving your doctor.
Basically, for any medication, the lowest dose that is effective is the right dose.
This seems to be working for your client.đź‘Ť
5mg is the dose Dr Mark starts people on, afaik.
Good luck.
