I take Mirapex every day at 3 pm to try to reduce rls from starting up. Seems to do better that way rather than waiting until evening to take it. That being said, It is a rare day that I don't have rls at all. I also find some relief in lying on my stomach. Adding gabopentin helped some too but I can't take that any longer due to an adverse reaction. I am on iron supplements as well as magnesium. As I get older it seems rls symptoms get worse.

Hi

if you're having to take pramipexole earlier to stop symptoms, it suggests symptoms are starting earlier.

If you're having symptoms anyway, it suggests the pramipexole's not working.

Unfortunately, put the two together and this sounds like augmentation.

Normally, I 'd suggest switching to gabapentin because the only real sure way of stopping the augmentation is to stop taking the pramipexole.

I understand that as you react to gabapentin, you can't do that.

In which case it seems you have limited options. One is to switch to a low dose opiate. It might not be an easy task to get a prescription, but I'm hoping other members who use an opiate might chip in..

I recently read of a member who found bupropion ( an antidepressant) helpful, as it raises dopamine levels. Assuming you live in the US. (Mirapex), it's known as Wellbutrin.

Sorry, if I'm wrong and you live in the UK, you won't be able to get a prescription for bupropion.

Another option is to switch from pramipexole to an extended release dopamine agonist, i.e. either extended release ropinirole (Requip) or Extended release rotigotine (Neupro). This sometime works for augmentation, but is not a brilliant option as all dopamine agonists can cause augmentation.
