CHC Assessment

My Mum has a CHC assessment very shortly was wondering if there were any questions/comments I should be making at the review. Am hoping that the care home know the procedure well enough to be successful first time. Or am I just being naive and get ready to appeal already! Thanks.

Hope the assessment goes your way.

From my experience make sure you/care home have up to date reports for SALT, physio, mental health team, GP or any other agencies that have been involved in your mum’s care.

It’s important to be able to show how the domains impact on each other. Example, swallowing problems affect speech. Slow cognition affects speech. Inability to communicate properly causes emotional and psychological issues. This changes day to day and within the day so is unpredictable. I think it’s good for evidence of this to be shown in the nursing home records.

The social worker said mum’s nursing home records were very comprehensive but family input is important and you may be able to add a different angle that they haven’t covered.

It’s a tough process, good luck.

Thanks Willowden, will try and remember to emphasise how they impact with each other.

it depends on the care home! Mum’s care home had experience of going through the process but not of actually getting CHC. As you’ll see from my comment in the link referenced above, the care home didn’t think she was eligible. And first time round the assessment was actually postponed because the home had not got all the relevant professional input despite my requests. I assume that every area includes a form for the family to complete. In my experience that is key. And always focus on medical/ psychological needs not personal care.

Thanks Messier, I did read how you had to push the care home and such a relief you received it. Will concentrate on psychologial needs and worst days. Just like the old DLA days for my son!
