Chc funding dst meeting

Hi everyone checklist has been completed and I'm told my dad who has cbd is eligible for the next stage a dst meeting. What should I expect and how should I prepare. I'm so nervous. I'm the only family member trying to help my dad.

Dear 144jim

It is really good to be prepared. Do take a look on the PSPA website it gives great examples of things to include. I found it really helpful as we made our application. Keeping a diary for a few days is also a good way of demonstrating need.

Make copies of OT, SALT, Physio, continence clinic, Neuro clinic letters/reports as additional evidence. Anything that records need will be helpful.

The Beacon website is also worth a look .

Agree with everything Tippy has said, diary is really important, PSPA website in their information for professionals area has a really useful document that helps with the overlap between areas, which I found difficult to put across on our 1st failed application and wished I'd had that document then.

Record what it's like on the worst days and concentrate on these, not the best, or the normal. Mum's GP said to say that though Mum's skin integrity was currently OK, because of her poor nutrition, she was at a high risk of developing problems, which bumped the score up in this area.

Have you got a health professional to help you? Community Matron, District Nurse? If you can get one to support you, they can be a big help pulling together all the other discipline reports and putting the information across in a more medical way - took me a while to change my language from "Mum chokes" to "Mum is at high risk of aspirational pneumonia because....."

Beacon do print booklets to support as well as their website, think you can ring to get copies.

Hi thanks. I don't know exactly who's going to be present or how many people I've not been told. but as far as I know I'm having a member of staff from the care home he's been staying in on a temporary basis since he came out of hospital will be there. I don't know if they've already made a decision and are meeting to tell me how they made their decision or if its a general discussion. I've got a copy of the checklist. I was present when it was done

Hi,

Aside from documents from Neurologist, OT, physio, SALT team etc. may I suggest you get your district nurse, staff or volunteers from the hospice who sees or takes care of your Dad to attend the DST meeting. The more people involve on your side the better.

Hi well I've had the meeting now and the only people I had was dads social worker a nurse from the care home and the chc nurse assessor meeting dad for the first time.she gave him the dst list.didn't give him time to read it.fired questions at him and me and have low scores.she said she was going on the information in the care plan and how he is at present.I kept saying his condition is being managed well in the care home but he's got lots of history of falls infections and hospital admissions etc.I felt she was playing down his condition.I tried to give her paperwork I had from his admissions but she wouldn't read it.I asked her what her knowledge is of his condition and she said she declines to answer.now she's put in recommendation to verifier for nurse funded care not Chc with my paperwork.dad wants to come home so he won't get any financial help in that case.
