In the US (different in Europe and Canada), so many men are referred for a first biopsy, and such a small percent have detected significant PCa, that mpMRI targeting doesn't make economic sense. Consequently, it can be challenging getting insurance to cover it. If suspicion remains after a first negative biopsy, or if insignificant PC is detected, they will usually cover it. Consider taking a Prostate Health Index (PHI) instead of just PSAs to help decide whether a biopsy is needed at all. Also, look at other factors - prostate size, family history, African-American, recent sex or bike riding, and PSA pattern before deciding on a biopsy.

For whatever it's worth, I have had 5 biopsies over the years (as part of the AS program at Hopkins). The MRI fusion guided biopsy was the one that gave the "best picture" of my PC and triggered treatment. I suspect (but obviously cannot prove) that the other biopsies missed some of the PC.

I agree with Tall_Allen's advice re other measures to consider.
