Any input out there on what PSA values should be six months after SBRT?

Lower than baseline, unless you have a history of prostatitis.

What T_A said echoes exactly what my RO said "lower than it is now".

At first the decrease will be more rapid, but it can take up to 3 years to reach Nadir or baseline.

Typically should be below your last PSA prior to treatment. The baseline I’m referring to is the lowest it drops after treatment.

Mine dropped from 5..4 pre sbrt to 1.6 6 months later, this was in line with expectations, can take a number of years to reach bottom line, next psa in March 25

Mine went from 1.11 to 0.48 after 6 months post radiation. Then 0.21 after a year, eventually reaching a nadir of 0.16 after 2 years. Where it stayed for another year so 3 years to reach a nadir is about right.

No, no ADT. I read the guidelines for favorable intermediate with a small amount of grade 4 and they allowed for radiation alone. Thanks to all for the input. I'm just in that window, six months post-treatment with recent PSA values in hand, and still 3 weeks out from the follow up with the RO. We just never had the discussion about what to expect the PSA values to be after treatment. But I guess 7.4 to 1.4 is an improvement. Thanks again.

ADT or no is a highly relevant question. You would probably be undetectable on ADT so your drop is good. Dr. Storey has a pretty good analysis of various studies on his substack and it looks like you are right where you want to be according to the one detailed graph. Note in that particular study graph patients did not take ADT

Prior to SBRT, my PSA was between 4.5 and 6.5 (see profile). Three months after SBRT, it was 1.52. Six months out, it was 0.92. As many threads have noted here, expected nadir is a few years out. I will monitor quarterly and update my profile.

As everyone's cancer is unique to them so are their PSA values after treatment. I'm not sure how important the actual number is. PSA is influenced by many factors. One such factor is whether or not you had ADT as part of your treatment regimen. I began a 6 month regimen of Lupron a month before SBRT. My PSA went from 5.1 down to less than 0.04. At 6 months and 12 months post ADT and SBRT is was still less than 0.04. I am now at 16 months and it has bumped up to less than 0.1. As of now I am not sure I have reached my NADIR. I suspect it will be higher than it is now.
