need advise on my current PCa situation

My question is for Tall Alen for his sincere advise on how to go about my situation - I was diagnosed with PCa stage 4 Gl 3+4 , no bone metastasis as per the Pet scan but lymph node was effected . I was immediately put on Lupron and casodex since there was lymph node involvement and ever since i have been on lupron every 3 months and casodex everyday - i have been monitoring my PSA every since January 2018 and now there has been a jump in my latest PSA which reads at 3.77 which was taken on 12th October . My request is to let me know what steps should i take going forward and what options do i have as my next step i am from india ... PSA readings in July 2018 0.819 / September 0.840 / Jan 2019 -1.007 / April 2019 1.013 / July 2019 1.633 / October 2019 3.74...Thanks

Just for the records:

There is this Salvage Lymph Node Dissection clinical trial now recruiting in Vienna Austria.

There is cost involved for non-Austrians.

In India, you should be able to get a PSMA-based PET scan to make sure that the cancer is only still in your prostate and lymph nodes. If so, you can talk to a radiation oncologist about curatively treating your prostate and pelvic lymph nodes. You might also be able to get darolutamide.

Hi TA.. Need your advice once again as I am really confused with different doctors suggesting different modalities for my PCa..one suggested surgery other suggested radiation etc etc with your vast experience I just need your opinion on my case on how I should be approaching my treatment plan... Although I am taking other opinions also but weigh your suggestion by far the best..

I need to share my pet scan report... How can I possibly do it with you... Best regards and thanks
