best hour to take pred

I’ve read somewhere it might be a good idea to take prednisone around 2am as inflammation usually sets in about 4am I have only taken for 3days but have found that when taking around 7am the symptoms do not ease for about 4/5 hours and I’m not sure that this is just the symptoms easing naturally or the prednisone kicking in - any suggestions please

In intro post I explained about the cytokines coming into play at 4am. .. which is why we sometimes suggest taking Pred at 2am.

But as you’ve only been on 15 mg for 3 days you really need to give that time to see if the dose is high enough..

It may not be as it doesn’t seem lasting 24 hours. But that needs to be discussed with your doctor - so please do.

I appreciate you want to feel the best you can, but it’s early days and you are still in the diagnosis process, so we shouldn’t really advise changing anything at this point.

I know where you are coming from, and feel for you, but you need to stick to what the doctor has said initially and then report what’s working and what’s not. 🌸

Oh 2am! Really recommend. No probs upon waking, just imagine the cytokines slowly dripping into that bucket! All best wishes.

Me too, 2am worked so well.

Definitely does!! The PMRGCAuk talk today was about stretching to avoid stiffness - never a problem for me.

I've been feeling stiffness during the week before the tcz jab. Stretching really helps, and a new, fabulous pill I found in a pharmacy in Athens: paracetamol + ibuprofen together. New to Greece, can't yet get it in Australia. 👌

Bet its cheaper to buy them separately - fine to take both together, really improves the result.

It's convenient and easier to take just the one pill, and not expensive. I think they're great. The weather has been good to us over the last 4 weeks. It's been great travelling around Greece and then living like a local for a week in Athens. Finally a week of island hopping on Richard Bransen's boat. Now heading home to winter Downunder 🥶

I take it RB wasn't around? Sounds lovely.

Haha, no. But, not being a cruiser, I was very pleasantly surprised. A fabulous experience. Now back to reality and an appt with my new, (young) Rheumy in a week. This could be interesting with regard to ongoing tcz. Perhaps fresh eyes may be good. We'll see.

A couple of nights ago I took my 15mg of pred at bedtime in error instead of the AdCal that I usually take at that time. The next day I was almost totally pain free so I did the same the following night and the same thing happened so I am continuing to do so.

A bit of trivia here. They like people to take pred in the morning because that's supposed to help adrenals recover more easily when coming off pred. But the thing is, PMR is not a quick fix so we're on it long enough for adrenal suppression to happen anyway. The other, more pertinent to us, reason is in early days when at our highest dose, evening pred can cause sleep problems. A dose taken at 2 am doesn't do that, provided you are able to wake at that time, take the pred with a bit of food (eg yoghurt or cheese or half a banana) and go right back to sleep. When eventually tapering and hoping to reach zero it's easy enough to switch back to breakfast time.

A few years ago i wrote a piece on 2am dosing. You should find it on the site. Scientifically it stacks up, and after nearly 7 years I still take my 3mg dose at 2am ish. Infact I now wake naturally and go straight back to sleep afterwards. For many, it is a game changer. As your name says....Gotryit!
