I have just recently had wonderful success with a ganglion impar nerve block; as nothing else has worked for me. After 5 long years, I have markedly reduced symptoms. Amen and amen.

thanks for the suggestion. I had the ganglion impar block, but it was not successful. I heard from primary care doctor that the medication above has proven to help with chronic pelvic pain. Rocky68

Chickymac, can you tell me where your pain is located. I have low back pain, cannot sit long and every time I lie down I feel rectal pressure (feels like you need to go into your power bitch you don’t) and spasms. I feel like I take medications every two hours for this condition. Your response will certainly be appreciated rocky68.

my pain is left side rectal pain, a pinching or twisting tight feeling; and often even a feeling like a raw open sore rectally. I could not sit long, stand long or walk any real distance. I have tried most all meds, PT for 4 years and so much more. Somehow the ganglion impar block worked for me.

thank you for your response. I may try the shot again. Rocky68

I’ve tried LDN and ULDN. Didn’t help me. I had side effects.

My sis-in law uses LDN (low dose Naltrexone) for pain and swears by it. Says it has given her hope she will get her life back. Not exactly sure what the origins of her pain are - disc or fibromyalgia or both. She said i should look into it for my husband's Pudendal Neuralgia. We haven't asked his doctors about it yet, so any info anyone has on this would be helpful.

I’ve asked my Dr for prescription but don’t know when or not I’ll get it. If I do I’ll post results. Pudental Neuralgia is horrible. Rocky68

I tried LDN. I was hopeful but it didn't help. You have to get it from a compounding pharmacy. I ordered from Care First Specialty Pharmacy. Ask your doctor to submit the prescription. Check the site for prices. It's pretty reasonable as far as compounds go. Good luck!

Southern Sal. After dealing with this for 12 years now I have my doubts about the medication working either. My pain is mostly located in the rectal area. I cannot sit very long and sitting causes back sacrum pain. The worse is laying down because I have such bad spasms and I feel pressure to go to the bathroom when I really don’t need to.

Would you mind sharing where you’re having your primary problems rocky68

Sit bones, perineum, and all along the pelvic bones. Sitting is my problem. Not one dang thing has helped even slightly. I'm going to start pelvic PT, again, in a few weeks. I also ordered kratom, which is controversial, but I'm willing to try it. It should be arriving tomorrow. I'll update if it provides any relief.

Hi SouthernSally, I just replied to your meditations message and then saw this. I also have chronic sit bone pain that seems to have been triggered by a disc issue. I get spasms in the lower buttock and rectal/groin area too. Just had a bilateral lumbar/sacral RFA which doesn’t seem to have helped. Pelvic PT checked internally and no major dysfunction. Cleared by gynae too. Grateful for any advice! Taking pregabalin and amitriptyline. Thanks in advance, am in UK.

What kind of doctor diagnosed the disk issue? Ultrasound can check sit bones for inflammation or ischial bursitis. I saw a sports orthopedist for that but he found nothing but a little high hamstring tendonitis which I already knew of from an MRI. But he didn't think that would cause severe pelvic pain. I'm going to return to a pain doctor and ask about further injections. I also have a telehealth session today with a gyno specialist I've seen and will ask her about a diagnostic laparoscopy.

Hi, disc issue diagnosed by spinal specialist. Am not candidate for surgery. Symptoms came on after an injury sustained during first Covid lockdown. Have double disc prolapse and bilateral foraminal stenosis, mostly on left side. Pelvic symptoms mostly on left, seen sports specialist too who sent me for high-grade pelvic MRI. Things like bursitis and tendinopathy ruled out. He said pudendal neuralgia, had the shot but did nothing for sit bone pain. He couldn't understand why I was in this much pain, or why back issue would cause the pelvic pain. Will continue to see pelvic PT, she is MSK too so will see how things go. She has already mentioned ultrasound, something I've never had. Swimming helps and I sleep OK. It's actually trying to live normally that's the problem! Had to give up work. Thanks for replying and good luck with your telehealth appt.

Has your pelvic PT checked your obtorator internus muscle?

Maria31 profile image
Maria31 in reply toSouthernSally

Not sure. Would she have felt that during vaginal exam?

SouthernSally profile image
SouthernSally in reply toMaria31

Probably but I'm not positive. When I see my PT again I'm going to ask her to check it. It's a large muscle that attaches internally and externally and is sometimes overlooked.

Maria31 in reply toSouthernSally

Thank you I’ll do the same. RFA on that side had “piriformis infiltration”, whatever that means, but don’t think piri is the problem! Hip is all tight on that side so what you suggests makes sense. Take care.

She said it was Levator Ani Syndrome. One doctor said it was prudential Neuralgia, another said Periformis and the one in Chapel Hill says it is the Perineum muscle. Honestly, I don’t think anyone actually knows. I have been to three orthopedic doctors, had the coccyx removed, and a stimulator for about two years until I had it taken out. I have to be real careful what I do during the day especially lifting heavy objects and squatting. The doctor in Chapel Hill, who does my Botox injections said she did not do perineum injections, because there were so many nerves concentrated in that area.

I’m sorry you’ve had to go through this. Can relate in that every dr says something different! Really hope you can find some relief.

Rocky68 in reply toMaria31

You may want to try muscle relaxer Methocarbamol (Robaxin) which helps with the spasms. I don’t take Robaxin if I am going to be driving however. Don’t let Dr put you on Baclofen because my doctor in Chapel Hill said that was for people with cerebral palsy. The .5 mg of Ativan helps me to relax also. I cannot sleep unless I take one Robaxin and at least one Ativan at night, I have had all kinds of shots including two caudal epidurals. A number of pudental nerve blocks, which did not work either. They did say I have a slightly bulging disc at L5. S1 and I am still not convinced that that is not really causing all this pain. Let me know if you find something that works. Rocky 68.

Maria31 profile image
Maria31 in reply toRocky68

Aha, thank you. Am coping on my fairly low level of medication, nut will defo keep Robaxin in mind. I feel my back is part of the issue but not the whole story. It's so frustrating. Good luck with everything. It's hard.

Rocky68 in reply toSouthernSally

Your pain is just like mine except I have very tight spasms in my buttocks and rectal area every time I lay down. I can control it with a medication called Robaxin but since it is a muscle relaxer, I do not feel like I can drive. My doctor has authorized me three Ativan a day which helps as well. Of course I cannot function without these medications. I also take tramadol 50 which is not really effective unless I take Acetaminophen 500mg I also take 150mg of Pregablin twice a day as nerve pains go down the back of my legs. Gabapentin did not work for me. Be very careful with the herbal medication I have tried physical therapy twice along with dry needling, and none of it worked. I think I’m just going to be on medication the rest of my life and do the best I can pretty darn depressing. Let me know what helps. Rocky68.
