Should I push for a test?

Update - I have realised that a few more results have come through.

He ordered dsDNA but apparently the sample was lost or there wasn't enough?

He ordered ENA Antibody test but the result says unknown?

ANA Test - Anti-nuclear factor level POSITIVE - Titre > 1:640

ANA PATTERN SPECKLED

In the presence of consistent clinical features, speckled ANA can be seen in a range of autoimmune and chronic inflammatory conditions.

Significance increases with titre. ANA and ENA change very little overtime, and have no role in monitoring disease activity. As such, there is usually no role for repeat testing unlessthe clinical presentation changes

Ferritin was 115ug/L

B12 - 382 ng/L

In the mean time I have been put on a hormone patch which seems to have made no difference. And I've upped my levothyroxine which also seems to have made no difference.

I am still knackered with brain fog and achey joints.

I want to do another Active B12 test soon, will read up on when to stop supplementing before it was I got a bit confused last time and think I stopped too soon.

Any advice on what the ANA results mean would be much appreciated - thank you.

Ellie

Hello all,

I asked my doctor for a PA test after getting a result of 44.2 pmol/L in a medichecks test, which he agreed to, but when I got the results through it didn’t seem to include the specific PA tests I was hoping for.

Its been suggested that it may just be because I’m vegan so I was wondering whether I should push for it when I speak to my doctor on Thursday.

I have Hashimotos and my grandmother had PA and thyroid problems.

I’ve including the test results in comments. I take Thorne basic complex and I was taking a separate B12 every other day but I’ve upped it to every day now. I stopped them 7 days before my test.

P.S - I’d stopped taking the supplements 10 days before the medichecks test.

Hi, only my opinion, but I wouldn't have thought 7 or 10 days anything like enough to get a baseline. Once supplementing you make diagnosis much harder sadly, made that mistake myself. Your folate is showing high, which could be a problem and could be useful information. If you are supplementing folate/methylfolate it may be that you are taking too much or didn't stop for long enough before the test. If taking folic acid, may not have stopped for long enough or may be you don't use it well and would do better with a methyl form. The B12 could be high enough for some but not all. Unlikely to get a GP interested though. The PA in the family could be enough to get you an anti-intrinsic factor antibody test though, although they are often negative, even in those with confirmed PA . Being vegan you will have a hard job convincing anyone that it isn't purely a dietary deficiency, but antibodies, homocysteine, MMA, could indicate otherwise as of course you could be vegan and have PA or other B12/folate metabolism issue. The supplements will start to change those quite quickly for most people.

Hi, thanks so much for your reply. I spoke to my doctor yesterday and he did actually order the intrinsic antibodies test and they came back negative so that’s that. I have upped my supplementing now so I’m taking 100mcg via Thorne and also a separate vit b12 supplement giving me many thousand times my daily amount - it should be coming out of my ears now and véganism shouldn’t be a factor - although I’ve always supplemented so 🤷🏻‍♀️

bookish , when I next do a b12 test (active is part of the quarterly one I do with medichecks for my thyroid) how long should I stop the b12 supplements for beforehand please?

If it’s still low after the next test what can I do?

Thanks again,

Ellie
