B12 deficiency, fatigue, twitching, anxious, insomnia

About three months back I was diagnosed with B12 deficiency 182 pg/ml after multiple doctor visits to find out my strange symptoms.The doctor took it very lightly and prescribed 1000 mcg methylcobalmin daily ,however symptoms that had started with tingling and jolts in leg and sharp low back pain shooting through leg developed into nausea, acidity and vomiting and extreme fatigue which took a month to resolve.Now I am experiencing painless twitching and sudden muscle jerks in legs and arms,slow movement and stretchy muscle behind knees which feels sore and stiff if I try to push myself through walking especially left leg,heavy head in morning, insomnia and overall slow movement which has made me really anxious and scared about possibility of other horrible conditions.Basically I have not been normal in last two months.I wake up with feeling weak in hands and feet with mild numbness in morning .10 days back my levels were 476 pg/ml.

Does anyone relate to this or advice on like if I should switch to B12 shots?

Some have s bad reaction to methylcobalamin. If anythi g is a reaction to the sort it means tiy are absorbing it though.

Have you had a referral to a neurologist ?

Some things you say have resolved so that is good in such a small time frame.

Have you a review with Gp as well?

Although your levels arnt very low sometimes that doesnt actually mean anything. Its symptoms and your response to b12.

Were youb given an intrinsic antybody test at the start?

Thiss can pick ul 50% of thoses with PA

Also did you have folate ferritin iron vitamin d tested ?

So you are absorbing b12 orally. as levels have gone up considerably. . You could always try cynocobalamin 1000 mcg tablets? Neurologist should see you though . Get a referral. As a long wait probably.

As for injections I personally would wait if improvements are happening. B12 injectiond wont do any harm though. K would discuss with a doctor. First one should always be done in a medical setting incase of a reaction. Rare but can happen.

. Repair can take time . Much longer than i ever imagined .

I have had a consultation with neurologist last week and he thinks I am ok just anxious which baffles me since anxiety should not cause slow movement .My all other blood work like cbc,crp,esr, magnesium,lft,rft thyroid, calcium ,vitamin d etc were normal.Its kind of cyclic as I tend to recover then again back to square one.

Your serum level of B12 indicates that you may have B12 deficiency. What is the cause? Have you pernicious anaemia? Is it due to another cause, eg, Long term PPI medication use?

If you have had the symptoms (and of course the deficiency) for years then the symptoms you are getting in response to the supplements, may be the deterioration which seems to occur in people with long-standing vitamin be 12 deficiency once they start treatment. This deterioration, as you probably know, can occur over months or even years before a gradual recovery . As far as I can see, no one understands why this is so but there are plenty of hypotheses.

Of course, as already mentioned, it might simply be that the methyl cobalamin does not suit you.

I have never used PPI for long term except for last one month and my GP won't test for PA since he thinks it's only 50% accurate and treatment line remains same which I doubt since I used to eat lot of chicken and eggs.

This walking issue really troubles and is freaking me out so does waking with a heavy head. My blood work is normal for other relevant tests.

As far as i can see you do not say how long you have had symptoms. Have you had them for years or only for a shortish time? This may affect your response to supplements.

I had somewhat similar episode some 6 years back but that time B12 was not tested and I had recovered within 40 days and never had any health issue again.This time the noticable symptoms started in July this year. Actually the symptoms manifested slowly and kept changing ,in July I had fatigue, tingling sensation in right leg and low back ache then in August it was lot of nausea, acidity and occasional vomits in morning and extreme fatigue. Most of the fatigue,nausea and vomiting is gone but leg weakness with mild pain while walking in left leg still persists and so does twitching and sudden muscle jerks which has started some two weeks back. Blood work like magnesium, potassium etc have come normal.

Should I switch to injections or just be patient and does twitching, walking issues are common in B12 deficiency.

i cannot think what else to say - others have said it. , Your GP is correct about the test for intrinsic factor. of course. But if it is positive you will know you have pernicious anaemia. If it is negative, you still will not know what is wrong.

Methylcobalamin made me feel very anxious,. Cynocobalamin has never made me feel anxious. Try another type of B12 and see if it helps.

Hi MokayD,I have thinking of that but have heard that cynocobalamin is converted to methylcobalmin only.

I don't know why, but I tried methyl tablets a couple of times and each time I felt just awful, anxious, sad, my skin felt kind of like crawly things were running over it, horrible insomnia. Within 24 hours after taking the tablet these feelings went away. I've given up on anything methyl. I initially tried them because i have some of those methyl gene deficiencies but I found out that that doesn't mean anything. Unless you have a double deficiency of one gene it isn't a problem.
