Hi, I have PA diagnosed October 2018 with full blown symptoms . I have started with cyano, Hydroxocobalamin initially which did not work so switched to methycobalamin. I need very frequent injections and it’s almost 1.5 years now, most of the symptoms subsided with few symptoms still present. I have severe acne which doesn’t go away due to methycobalamin. So switched to hydroxocobalamin from last week with 3 injections and all of the symptoms are back again and I feel now. I am not sure what is this problem with hydroxocobalamin with me. Do I need to continue with methycobalamin or try hydroxocobalamin for few more days. Do I have some other problems? I just went through this article and don’t understand as they talk something about MTHFR.

Thulasi1 - I wouldn't pay much attention to the article and I certainly wouldn't worry about it. People do vary in how they respond to B12 and different types of B12 and although there are probably genetics involved in that the number of processes in your cells that involve B12 means that the table is extremely simplistic.

MTHFR variants tend to have very little impact on the way B12 is used by your cells - most variants have very little impact at all and if they do then it tends to be on the way folate is used.

The best thing to do is to go with what works for you and if methylcobalamin has worked for you then stick with that.

Unfortunately, it is unlikely that switching the type of B12 is going to make any difference to the acne as its usually a reaction to the B12 itself - the micro-organisms on your skin react to the higher concentrations of B12 by secreting a compound that your skin finds toxic - hence the reaction and the acne. Likely the best thing to do in this instance would be to ask a pharmacist for a recommended skin wash.

Hi Gambit62 , thanks for ur reply.. I have checked my Folic acid and iron levels, they are all in good level. But I sometimes cannot breath properly during night times and I get a burning pain all over body if I continue to breathe forcefully. I am not sure what is this symptom and I even get heart palpitations. It already 1.2 years of 3 days once methycobalamin injection. Some of the nerve problems subsided during this time and I am atleast happy for that. I don’t know if I have folate obsorption problems. Do you or some one in this forum know if I have to do some specific tests ?

there is no actual test for folate absorption that I am aware of. supplementing folic acid (may be try a methylated form) is likely to be a good idea if you have a B12 absorption problem but avoid very high doses - definitely don't exceed 1000mcg/1mg if you are supplementing.
