It sounds like postural hypotension. Has he seen the doc? What's his blood pressure?
If his heart rate gets faster and he feels like he is going to pass out. Tests can be done . A standing test is done e when your heart rate is taken and you stand on the spot for so long(cant remember his long) also he will hopefully get a 24hr bo machine. To see what is happening. B12 can affect tour autonomic function so needs to see a doctor.
Meanwhile try drinking alot of water before moving and clenching his calves by crossing his legs.
Dont stand in the spot for too long.
One of my grown up children went in to develop pots with undiagnosed b12 deficiency/PA.

as fbirder says - it sounds more like postural hypotension - basically when he stands up his blood pressure drops because the heart takes a while to adjust to the change in gravity and it gets momentarily less efficient at delivering oxygen to his head.
It may be that his pressure is on the low side to start off but its also something that tall people are more prone to.
Being careful to stand up a bit slower may be enough to stop it happening but if the speed with which he stands makes no difference then he should definitely go and see his GP.
He needs to take his pulse before moving and then only move very slowly, taking his pulse as he moves. Only move once his pulse is steady. I have to do this every time I get up from sitting or lying. You do get used to doing it.
This is even fairly common with highly trained athletes who have very low pulse rate. My son, an athlete, says he is blind for around 3 seconds after standing up from the loo.
