Hello, I was just wondering if anyone has found a good template for a symptom diary that makes it easy to track symptoms over time? I'm imagining a spreadsheet, but would be interested in anything you have found that works well for you.

That's what I'm using. If you PM me your email address I can send you mine. I print it out and fill it in manually.

My excel sheet template is available here for anyone who wants to download it:

(It can be imported into a google sheet if desired.)

You'll have to adapt the symptoms for whatever's relevant to you.

I just had a notebook readily accessible. It really is what works for you. Spreadsheets allow you to track multiple symptoms.

Remembering to fill it in and exercising the short term memory to fill it in are also two key ingredients.

I can't remember what I was searching for once but I did stumble upon some health related apps that are intended for tracking symptoms. I don't know if you want a smartphone app, but just thought I'd suggest it in case that's what is easier for you.

I do a spreadsheet (not a particularly good one, but it's good enough for me). And I keep it on my google docs so that I can fill it out wherever I happen to be. The only downside to google docs is that you have to have not just the google drive app, but also the google sheets app in order to be able to make changes to the file.

oohh i'd like a smartphone app! I had looked for one a while back but didn't find anything useful. I did a quick search just now and found one that looks suitable but it's for apple (Symple) and I'm an android user. Most others seem to be for tracking physiological parameters like BP HR and so on.

Symple looks like a good find jade_s. Free app if you want to track 10 symptoms - can upgrade to unlimited number of symptoms (and even write your own instead of selecting from a list- a must 😄) - cost £2.99. Can also export information as a csv file and import into excel etc. to make lovely spreadsheets 🆒.

Going to have a play with it properly at the weekend and get the upgrade if it makes good xls.

I use T2 mood tracker. It was originally developed for physiological symptom tracking, but you can adapt to any scale you want. I use it for brain fog, heart palpitations etc. It has sliding scales so takes about 30 seconds to record and gives you a graph of symptoms up to a year. Haven't checked whether it's android as well but I found this worked well for me
