I have long term ongoing pain linked with fibromyalgia, costcondolitis, sciatica etc.
I woke up this morning with excruciating pain just above left elbow, travelling to fingers, but it intensifies when I cough or swallow, causing pain left side neck and left shoulder blade. Does anyone have an idea what this could be. ??? Pain is intense.

This could well be a shoulder or neck problem, as many of the nerves run through that area. There is also a muscle that runs between your neck, down your upper back to below your shoulder blades, and if you have a spasm or tightness in any of those muscles, or if you have pulled one of them, it could cause pain in other areas. I have problems in my thumbs when my neck is bad.
It might be worth seeing your physio if you have one or going to see your GP.
