Some years ago I had decompression nerve surgery on my left arm. I have had no problems since, about a week ago I was getting a horrible pain in my left arm and if I bend it for any length of time, even just holding a cup I have to strIghten it out again as it is so painful. The arm is tender to touch but mostly it is the inner part of the elbow. Could it be arthritis? Any advice please.

This is an emergency case. When you feel pain that doesn't stop in a day or two, you need to go to the emergency room to check it out. Don't hesitate my friend. Good luck.
See a doctor. Then see a chiropractor. The chiropractor can give a sensible second opinion.
