I have severe pain in my head when I sneeze or anythig strangeness. I just had an MRI and MRA completed with no oblivious concerns.

I do have tightness in my neck

I think you might find that you neck problems are all part and parcel of the severe pain in your head. Perhaps some kind of muscle relaxant or similar to ease the tightness in your neck. You might find an improvement with your head. I have very similar problems with my neck C3 to C7 narrowing & facet joint arthritis which causes tightness & head pain.

Perhaps a return to the docs or referral to pain clinic

I have same problem coughing, sneezing, and ear hissing and mine stems from neck and head , and don't think these mri's are an exact science, so do not take their word that there is nothing wrong because I had so many MRI'S before they admitted that there was a problem in my neck and still can't get it in writing from them, so don't give up as it is a sign of something in that area I think and it can get worse believe me it develops into a severe problem (if it is like mine that is) but I hope not as you don't want what I have in my neck, best Alex

It is possible that you have two possible problems that do not show up on x-rays or MRI/MRA. These are muscular and postural. You need to check if the muscles in the neck, head and upper shoulder are not too tight. A chiropractor can check for this. The postural arises because heads are heavy. If the head is not balanced on top of the spine then muscle is needed to hold the head up. This can cause some muscles to become over contracted. If the head is off alignment then the eye muscles have to hold the eyes in the horizontal plane. This in turn could cause muscles to become over contracted which cause pain when you sneeze.

One can only investigate if I am drawing a correct or incorrect conclusion.

Do you, maybe not have obvious, nasal problems but do you feel 'all blocked up' sometimes? A friend of mine suffered this, for years, before she was found to have a 'polyp' obstructing her nose-at the back, where it was not easily seen. She tells me 'things' are much better now.

The ENT did ID a polyp but it is not obstructing air flow.
