neuro has given me scrip for modafinil for chronic fatique [have primary progressive ms] read reports and not sure, she also mention amantadine ? ldn doc will give me script but it is a 'off the shelf' med ?? just wonder

Low dose naltrexone has definitely got quite a good following from people with MS as well as a variety of other autoimmune conditions. If you haven't already done so, visit the main information websites for it. I believe it is starting to be researched a bit more, even though as you say using low doses of it is still an "off-label" use.
Just be aware too though, that there are a load of meds out there being used "off-label" but very commonly - for example using amitryptylline for neuro pains and pain modification has been an "approved use" for quite some time, but using most of the other antidepressants for the same thing is still "off-label", and yet how many folk out there are being given other antidepressants for pain modification?

I have MS and was on LDN for a while. It didn't make the blindest bit of difference to me, unfortunately.
