When my acid reflux is quite bad it really hurts between my shoulder blades??

Congratulations on being a mother.
The pain could be associated with acid reflux. And possible with the post delivery muscle relaxation, pressure of the stomach etc. Believe this is acute side effect from the pregnancy and not related to heart etc.
Seek your primary care advice for prescription. However off the counter gaviscon advance can soothe the oesophagus for few hours.
Lying flat will also cause heartburn (due to acid reflux) especially if you breastfeeding. Tey and see if you can lie down with upper body slightly elevated by using pillows or inclined sofa etc.
Whilst your sleeping pattern is disrupted to match that of the baby, that also triggers acid reflux and associated pain. So please do speak to your primary care to seek medical advice as you are possibly breastfeeding your newborn.
