Should PPIs be stopped when stomach bugs get a grip?

Since increasing omeprazole by 10mg ( 30mg ttl ) have had mild diarrhoea the odd day or two, but now had explosive squits for 7 days. Stopped omeprazole about 5 days ago, not even getting acid reflux now, but also no appetite and whatever I eat turns to water and is expelled about 4 hours later. It's horrible urgh.

Just started some Imodium, no change after 4 tabs so far.

Sounds like you need to consult with you GP to rule out serious GI infection by analysing a stool sample.

hi

I have had this since my operation in 2013. I have been on 40mg of omeprazole since the op I still get reflux, again depending on my day. I was put on Creon and Imodium, the creon I take every time I eat, depending what I eat depends on how many I take, but I can be taking up to 32 a day!, the Imodium I take after every loose stool. Normally the Creon works well but recently ( last 5 months or so ) they havnt been working so well. I can go for a few days fine then , for no reason I have explosive tummy. It is foul in all manners. I have tried different dosages but just can’t seem to get it right. I am still under the hospital because of it all and even they can’t give me answers. I get quite down with it sometimes, I just think it’s the way we are now. I have had to take co codamol for migraines recently , which when taking them is great but once I stop them it’s a nightmare on my tummy.

It might be worth having a chat with your GP or Macmillan to see if they can prescribe you the Creon.

Although it’s not nice, it’s nice to know your not alone.

Keep us updated with your progress if you would.

Hi June, sorry for the late reply. Sorry to hear about your difficulties getting the right dose, it really is horrible never knowing when it will explode. Eventually the Imodium worked, a little too well tbh and had the stools tested but by the time I was able to take them ( without soiling myself ) it had cleared up. Also tested for H-pylori, but I do also have diverticular disease, which I read can leave you subsceptible to infections, but it never bothered me in that respect until the increase in PPI dosage. The nurse suggested IBS, but it seemed to coincide with the aftermath of a mains water leak. Switched to bottled water now.

Never heard of Creon, I'll keep it in mind if it reoccurs thanks.

Tummy's still a bit delicate, but hopefully all's returned to normal, hope the same for you too.

All the best.

Hi there pffft,I'm glad all has settled for you now, nothing worse than that fear of accidents. The Creon just slows down the process of food moving through to fast, doesn't always work tbh, I can't take too many loperamide as it has the constipation effect. I tend to take them more when travelling, just to be safe. Sometimes just seems to be more and more tablets to take. I honestly thought after the operation that I wouldn't need omeprazole again, no such luck. Food types play a huge part with me too, anyhow, we're still here and that's the main thing.

Take care

June.

I never had that response from omeprazole. We are all different! If it is a stomach flu, you probably aren’t eating much. You still have acid from your stomach coming up though. I’d probably continue to take it, simply because you already feel bad, adding to that wouldn’t be fun. If you think it may be the cause, lay off for a few days and see if you are better. Hope you feel better very soon!

Thanks for the reply. I should imagine nothing is more effective than stomach acid for killing off all the nasties, but honestly when it started I just didn't need any PPIs, I knew I was getting better when I started getting reflux again so started on 20mg, the bug resurfaced for a few days, Imodium helped, then started on 10mg, now I take 10mg every morning and 20mg before bed. Thanks, all back to normal now I hope.

Still get problems with loose motions at times but much better since taking creon. Not happy about lanzoprazol but reflux worse without it. I am 6 years from diagnosis. Surgery Sept 2017. Have come the conclusion it's something we have to get as good as we can and then live with it

Thanks for your reply. They tried me on Lansoprazole, gave me stomach cramps and yellow stools. Read that it can slightly increase the instance of stomach cancer, Omeprazole, not so much. Hope your surgery was a success, and yes, unfortunately, there's always worse things that can happen.

I am 5 1/2 years post surgery. I take 40 mg of pantoprazole once a day. Zero heartburn.

I tried stopping or cutting down my PPI about 4 years post op but a routine endoscopy a year later shows I had developed Barratts. I had no pain or reflux after about a week and thought all was fine. I am now back on 60mg a day. Do not want cancer again. Just make sure you do it under supervision.

Sorry to hear that, hope you're stable now. I was diagnosed with 5cm Barrett's 8 years ago which has never been properly monitored. My 2nd gastroscopy, 2 years ago was a disaster, was left with a hiatus hernia and the operator never even bothered taking a biopsy as she couldn't find it. Next time I'm asking for an ultrascan.

Omeprazole always gives me explosive Diarrhoea changed to Lanzoprazol which I found better, I’m now on 40 mg of pantoprazole a day taken early one hour before eating, it works for me.

I very rarely get reflux which in my case I believe is more to do with over indulging.

Could not live with Explosive D never could tell when I’d be caught short and the toilet smell is dreadful.

Had a suspicion that Small Intestine Bacterial Overload may be the problem.

Thanks for the reply. Never tried pantoprazole, I'll keep it in mind.

I've been okay on omeprazole for many years, but I did prefer Ranitidine until it was banned. I think the fact that the acid is suppressed leaves you subsceptible, no matter what brand.

Read that the Japanese are developing a new drug which isn't an PPI, can't find my paperwork on it atm.

I'll look up SIBO but personally I think the water board poisoned me.

I’m of 40mg omprazole. One and night and one in the morning. I also take one Imodium BEFORE I eat if I find my stools are getting loose. I also take 2 bio kult at breakfast and dinner.

Every now and again I take another Imodium if stools are loose.

I am now able to do the thing we all wish to do. Eat and drink at the same time! Albeit with a slight tummy grumble. No dumping!

I do not eat sugary foods straight after a meal to avoid bloating and dumping.

I hope some of above is useful for you. x

Thanks for the reply. Sounds like you've got it well under control, good for you.

Avoiding sugary foods is good advice, they do tend to ferment in my gut and give me wind.

I’ll never quite get it under control, it’s more a case of managing symptoms. I’ve found Imodium a few days will slow my gut, allowing the correct bacteria to grow in the correct areas of my gut, when combined with bio-kult.

My hypothesis is that each time I have loose stools, I’m also flushing out healthy bacteria, so they need to be replenished. Since taking bacterial supplements my stools are less pale and contain less fats.
