Help with ESA and Health form?

Can anyone advice me how to fill in this form please, I can't find the blog that had the link to helping with forms.

I Myself, made an appt and went to my local CAB and they assisted in filling the form out with me as it is difficult for me to write hope this helps

Lisa has given you the best advice available. You will have to make an appointment though. Gather as much documentation as you can lay your hands on. All your drugs that you take, drs names and rheumys as well. Sylvi.xxx

I've had really good help from CAB - make sure you ask for the disability welfare person though. In many areas they can actually do home visits if you have difficulty getting in to them.

go to FibroAction site, click on fibro and benefits this should take you to the benefits and work link where you can download the pdf booklet that helps.

on the vascilitisuk community site you can use the tags to link to the "route map to vaculitis which you can download. there are sections on symptoms and medcines that I printed out and sent in.

Hi

The best advice is get along to CAB or another welfare rights organisation and ask for help filling the forms in. If that's not possible I'd advise you to fill the form in giving a clear picture of you daily life and how the condition affects your functioning.

It's important to remember ATOS/DWP are not looking at medical conditions and medications you have, they are only interested in how you function on a daily basis. Also, if you suffer side effects from medications it's important to say so too.

I've always found benefits and work site really helpful and often advise people to have a look when claiming disability benefits. There is a subscription however, there's also lots of free information and advise on how to claim/fill forms and appeal decisions. I've added the link below for you.

Thanks to all for your replies, I spent along time trawling through them yesterday and have heeded your advice with regards to getting a third party involved, though with caution as when I asked for my DLA claim to be looked at again, a colleage who helps others with benefits said she would write me a letter of support, only problem was she wrote Appeal in her letter that arrived before mine and they didn't even read mine that was 4 pages long and although in their post paid enveloped was underpaid and they had to get pay for it! they wrote to me and told me this letter needed to be on this official paper, they had put my colleagues name on it instead of mine, error after error. I phoned and was told that an appeal would take 18mths but after looking at my paperwork he could see no reason why they couldn't look at the decision again, so that's where I'm at, I had a letter come that said I will hear about 11 weeks time. then I got ESA and was told that I am in a support group be the person on the phone because the letter didn't say, now I have this medical form to fill in, which after the day I had yesterday should be a doddle. I only went to the post office by car and parked 50 yds away But oh boy! I could hardly get back to the car, some how my legs decided they didn't belong to me, I was afraid as the knees and hips joints were like worn out elastic I felt like a puppet on a string with a few of the strings missing, very strange sensation. How do you describe that on an official form that asks can you walk unaided 50 mtrs? anyway less of my moaning and lets get back to the form.......

simple answer Carol you can't WALK 50 metres. walk is a normal regular movement that a healthy person does. a struggle is not a walk. I fought against accepting this, but sadly for me it's true.hope that's not too blunt., just trying to helpful.
