A massive pain in the bum! 🍑

Hi peeps, I hope this post finds you warm and as well as you can be today 😊

I am posting today, as I cannot seem to shift a reet pain in my posterior! It's sort of an all round pain in my right side: hip and deep in my buttock. I am currently taking a course of Prednisolone: 30mg/25mg...etc. 7 days each and reducing by 5mg each week. My FBC was normal, along with my CRP. My GP didn't do ESR, which is the only one I expected to be raised.

Two weeks in, and I'm no better! Usually Prednisolone sorts me out. Ì can't walk without extreme pain. I am taking regular paracetamol. I don't have back pain.

Any top tips?

I have similar pain in my right buttock, and all down my right leg, an intense gnawing pain. I’m certain mine is due to severe spinal stenosis which I have. I do find gentle stretches help, when I can get down on the floor, and heat pads ease it whilst it’s on. I’m currently on high dose prednisone for a chest infection & exacerbation of asthma, and like you it’s not helping that pain. I have ankylosing spondylitis, and 50% of patients never have raised markers to be guided by, I’m in that group, but the inflammation has still continued to cause damage. If it’s a nerve related pain, then meds like amitriptyline or gabapentin help.

I'm sorry to hear that you have a similar pain.

I have positive Rheumatoid Arthritis, which is kept at bay with. Tocilizumab and Methotrexate (Metoject) injections. I had a really stressful day at work, and it kicked in that weekend; there was no niggling beforehand.

Unfortunately, I don't get on with amitriptyline or gabapentin - I spent some months going through them all, way back, in the pain clinic.

I'd be interested to know what stretches you are talking about.

I’m normally on the biologic Benepali, along with my MTX, and life long steroids. I had foot surgery 7 weeks ago, which prevented me doing my normal floor stretches. I’m sure that didn’t help, plus I had to miss my immunosuppressants for 4 weeks, back on for 4 weeks, and currently been off them 2 weeks due to the chest infection.

Here’s a good nhs site, that shows you the stretches on short video clips. I do most of them. Stress is always a trigger for my pains. Some of these can be done on the bed, rather than the floor.

A few years ago I thought I’d cracked my hip bone, it was agony and I rang 111 sent to A&E and my back felt wet. It was Shingles so perhaps it’s not related to RA but bursitis or something else. I found ice helped although I spent a few days in hospital on an air bed for relief. Pregabalin helped but even now occasions it returns even though I’ve had the vaccine. So I sympathise and hope you get some relief soon.

Hi, thanks for your reply 😊 Poor you! You must have been in such pain!

It was thought I had shingles a few months back, so I had a course of antivirals. I have also now had both courses of the shingles vaccine. In hindsight, I think it was a reaction with etoricoxib and the aftermath of radiotherapy treatment I received after having breast cancer.

Unfortunately Pregabalin is another med that doesn't agree with me.

I'm sitting here and it's really burning inside my buttock 😢

Shingrex is 74 % effective and I’ve had it twice since the vaccine ! These things if they didn’t make us laugh would destroy us. I didn’t find any problems with Pregabalin butt (lol) I’d have eaten glass it hurt so much. My GP then told me to keep taking the Ayclovir and that’s worked, thank the Lord. Just another pill to add in the box. lol I hope you can get some sleep with it. I don’t know if it’ll work but Vick was helpful to me once the scabs healed. As was the Ice spray so might be worth a go?

OMG! You made me crease up!! 😂😂😂😂😂On a medical note, I don't have any external rashes, spots etc. Not that I can see any way! 👀

I have a mild pain in similar area. The past few mornings I’ve struggled to walk (when out with the dogs) but there is a big rock and if I make it that length and sit on the cold surface it’s much better afterwards. Really weird but you’d do anything to ease it. … maybe try an ice pack, I’m not suggesting you come visit to try out my rock. You’d be more than welcome and there is a lovely view from it but it’s probably not that practical.

I went to a &E with this on Thursday! GP surgery I'm trying to register with sent me there in case I needed x rays. ( I moved to another area a week ago). They didn't x ray but said it's sciatica, I'm not convinced. I've had sciatica many times but never like this. It starts in my right buttock and is so painful at the top of my femur near my hip, goes into my groin, travels right down the outside of my leg, into the side of my knee then down to my ankle bone on the outside. I've had it a while and then it eases but this time it's been 2 weeks or more. Nothing seems to help it. The top of my femur keeps getting stuck or gives way. I wish I knew what it really is! Trapped nerve they say...I'm not sure. If you find out what it is please let me know. It's driving me mad and wearing me out. A new neighbour of mine said it could be Trochanteric bursitis as she had it. Now she's waiting for a hip replacement.

When I had hip pain due to bursitis in both hips the only thing that helped was ice and gentle stretching, also massaging the area although had to grit my teeth for that. A steroid injection in my hip also gave some relief for a while. I don’t think I’ve been free of bursitis for almost a year but it’s more manageable some days than others. If you can a short walk can help even though this is very difficult at times. My hubby uses deep heat for hamstring and sciatic pain. Hope it eases soon for you.

I’ve had pain deep in the back of both buttocks for about two months. It’s pretty bad in the mornings and I find it hard to move around.

I had a brief session with a physio last week and she said she thought it might be inflamed glutes’ tendons. Sciatica was ruled out as the pain does not extend down the legs.

She said to keep moving but not exacerbate it. I asked if RA related and she didn’t seem sure, just said maybe. She said it could take two or three months to get better.

Hope you get to the bottom of it

I've had Piriformis Syndrome which sounds a similar pain in the butt! I did have some physio but in the end I think it was just time that fixed it. I think Periformis and sciatic nerve pain can be hard to diagnose because everyone's experience of the pain and it's exact location varies. If periformis syndrome hasn't been suggested it may be worth googling it as there are some things you can try yourself that may help. Hope it improves soon. Best wishes. Sebastian.

Could this be osteoarthritis in the hip on top of everything else? I once had this weird thing where I couldn't sit crosslegged because one leg didn't abduct (move away from t he body). I am naturally hypermobile and it was just really weird, no pain, I just couldn't move it. Like someone had built a brick wall I was pushing against.

Then one day my leg went out from under me while I was filling a watering can from the pond, and I nearly fell in! I was in agony. I managed to drag myself to the house to get my son by doing all that breathing stuff they taught us to do in childbirth.

It took a few days for the pain to subside, and then I suddenly noticed, I could freely move my leg again! I never found out exactly what it was, and the first physio I saw was months later and useless. But a different physio suggested it was the start of osteoarthritis and I might have had a bone spur, ligament tear or something else, but that the start of osteoarthritis is often the most painful time, then the pain tails off.

A few years later I had even worse pain in my other hip, and I lost the ability to lift my leg. I only realised that when I tried to get into the car and most of my body followed me in except for my leg! That pain lasted for over three weeks, and I used three hot water bottles, ibugel, and rest and paracetamol. I tried a bit of naproxen but that gave me headaches and codeine sent me off with the fairies leaving my gastrointestinal system behind in a prison. What happened this time? Again, nobody knows! (said with a comedic echo voice) But luckily I haven't had anything like that since.

I hope yours passes quickly. I found wearing my old maternity dungarees with hot water bottles stuffed down them was helpful.

You could try some exercises for sciatica:

I found the first one particularly good for the area you’re talking about. You can do it facing a wall and put your foot on the wall to make it easier to keep in place.

Hi there. I too had something similar , felt like burning sensation. In the end I went to see an osteopath and thank goodness she is fixing me. I was very tight all around sacrum and iliac and deep massage was a huge relief . Mine seem to also flare up when I’m in a little flare too but I do stretches every day. Worth every penny for me or if you can perhaps see if you can get referral to Physio from RA unit in hospital which I also am getting. Good luck ☺️

I've had RH butt/hip pain on and off recently. Mine is probably a lower back issue from an old gardening fight with a defiant shrub. Seems to be worse when I drive a lot because of right leg being stretched more than left. Saw a chiropractor. That wasn't fun. Resting seems to help. Hope your situation improves!

I've had similar pain that was a bad case of sciatica. Couldn't sit for long and the constant ache was similar to a toothache down my butt into my legs. Have you tried massage therapy? I had a great guy who worked miracles! Can't help to go that route and try. Good luck!
