Deejojo,
I believe that a very badly damaged kneecap can be removed. Your thigh muscle than has to work without the use of a giant ball-bearing, which is how the kneecap functions. But I expect it would be a major pain-relieving procedure even if walking is initially difficult?
Just a thought.
I have osteo in patella and inside knee joint. I found a 12 week targeted physio class at the local hospital very useful. It was very important to me to be able to strengthen my core, bum and leg muscles on proper machines and supervision. I did exercise at home before, but you just don't get the same results. Is there any physio available where you live? I hope you find something to lessen your pain.
I'm really sorry you're suffering. Thinking of you x
