The Care Quality Commission will be inspecting the services provided by Norfolk Community Health and Care NHS Trust from 15th September 2014. Services being inspected include adult respite care, podiatry, sexual health services, community nursing support, dental services, community hospitals (such as Dereham), health visiting, outpatients, palliative care and physiotherapy.

The inspection team is very interested to hear experiences (both postive and negative) from anyone who has used services in the last 12 months. You can send reports, peoples stories of care or other evidence your group holds about people’s care to chinspections@cqc.org.uk (include 'Norfolk Community Health & Care NHS Trust' in the title), by post to CQC, Citygate, Gallowgate, Newcastle upon Tyne NE1 4PA or by calling 03000 616161. Responses must be received by Monday 1st September 2014.

CQC will use your information to help plan the inspection and what to specifically look out for. You can read more about the inspections at cqc.org.uk An inspection report will be available after the inspection and published on their website.

Please do consider participating if you have experiences of this NHS Trust as it will help to improve services in this area.

Best wishes
