We have managed to have holidays every year for the past 9 year, in Europe. Everything is gone well without any problems with a normal family travel insurance.
We have been to countries with hospitals that they excepted EHIC card and then look for hospitals that they do holiday dialasys. But with COVID, this year was cancelled!

International Medical Group (IMG) will provide you with travel insurance plans which include medical evacuations back to the US. These plans are not very expensive and you pick the number of days/weeks of coverage needed. You can sign up via the internet, travel agents, health insurance agents, etc. The IMG folks will guide you as needed when you're abroad if you need help or advice. My husband has sold many of these plans to international travelers, including choirs and bands and more. My husband says the plans cover visits to hospitals and also the cost of medical evacuations, which are incredibly expensive as specialty planes are required, etc. Cruise ships are treated as foreign entities too. When we travel abroad we always purchase these plans whether our health is great or not. Hope this helps.
