Recently noticed a fall in Egfr from 58 to 46. Jumped on a VLPD, see how that goes at the next blood test. Is two weeks long enough to muster a change in egfr with diet? Creatinine is 1.2, Egfr 48 at last test 7/7/23. Should I wait a week or a month? protein intake has been below 45 most days. Would like to find a good renal dietitian. In my small town there are none, good or bad! 76YO. female. None of the other numbers are out of range except WBC count is low and RBC count is high. Any suggestions on a dietitian or time to wait for next test?

Wow, lots of questions.... no, two weeks is not enough to have a true impact on lab results. Not sure what VLPD means unless it is vegetarian, low protein. Thats a good one for renal protection. If you are hoping to increase your numbers, they may not depending on the cause of the disease and condition of the kidneys. However, it may stop the decline and that is terrific. There is a lot of information here on the is site and we love to talk food.
Something to consider is that as we age, our kidneys naturally decline in function. So what might be stage 3 at 30 is not the same impact as stage 3 at 76. The doctor should have explained that to you. So if your numbers stay around that level, and I would not test again for at least a month or more. And as long as you are not symptomatic.... well great! You said nothing else was out of range on your labs except high RBC, (cell blood count) but it might be only because you were dehydrated..... and low WBC, which is white blood cells which fight infection. Have your doctor review these with you, but again, could be age related.
The true test for CKD is over the long haul, meaning repeated testing over time to see if there is a decline, and if so, how rapid. You are wise to switch up to a good renal diet. See your doctor can recommend one or if there is a dialysis center locally, they should have a dietician on board.

At least for stage 3 CKD like yours, there's significant disagreement as to whether Very Low Protein Diet (VLPD) or even LPD is beneficial. And, of course, there's a risk of other issues eg muscle loss.

NB A renal dietician is likely to disagree with some/all of our strategy (and the scientific paper I sent you 8h ago) but we follow a Nutritional Therapy approach instead for 2 reasons I can share. We have found that approach to be successful so far🤞
