I wanted to get anyone’s take on something I’m going through. My nephrologist started me on Allopurinol because I was having gout flares before. I’ve been on allopurinol for about 1 week now with no symptoms and all of sudden last night I’ve got the worst flare I’ve ever had. Has anyone experience a gout flare after starting allopurinol? What have you done to lessen the symptoms? Any help would be appreciated!

You didn't mention the dosage you are taking. Inform your nephrologist about the situation and he might be able to increase the dosage. I've been on a maintenance dosage for three years now without any issues.

I too am on Allopurinol, for hyperuricemia and have not had issues. It did lower my acid level quite well and stopped the uric acid stones I had.
One week is not enough for the drug to properly take hold. I would also look at the foods you are eating and what you are drinking. Learn what has purines in it. Not eating high purine foods will make a huge difference.
My husband has been taking 300MG of Allopurinol on tablet in the morning for the last 2 years and it has significantly reduced the occurrence of his Grout Flare Ups. When he does have the Flare Ups his doctor prescribed Colcrys .06mg as needed. He takes 2 and within the first hour his flare is gone.
He has not experienced any side affects and it his kidney function, while diminished, has been stable and these medications, for him, have no side affect.
Yes, I had the same thing happen to me. My dr. said it was not uncommon to have a “breakthrough flare-up” shortly after starting allopurinol. I took colchicine, but it didn’t help that time (apparently it’s not as effective once you start allopurinol - although not2worry said it worked for her husband.) I was traveling with a tour group so just struggled through the week.
After another flare several months later, the dr. prescribed prednisone and it worked right away. I could only take it for 2 days as it keeps me awake all night. Good luck to you getting it under control!
I'm late in replying to this gout problem, but I want to suggest that you check your lab reports for the uric acid level. I had one attack several years ago and I never want another. My uric acid level at the time was high at 9. I was put on Allopurinol 100 mg daily and the doctor watched the lab results. this dosage decreased the uric acid a little, but he decided to increase the dose to 200 mg daily. This worked. My uric acid level has been stable now under 6 for a few years. However, my kidney doctor just recently started to look at that 200 mg daily and said he may be considering lowering it soon. As far as dealing with the pain of the attack, nothing helped me. But, others who replied had some suggestions. Also, I've heard from several other family members who also have gout, that attacks can occur frequently during the early part of starting on Allopurinol. Hope this information helps.
My uric acid was a whopping 12. I was getting flares every week. I would be in a continuous cycle of using steroids to deal with the symptoms. I tried allopurinol but was told to get off it after I had a flare.
I finally was referred to a rheumatologist who recommended Uloric but because of the black box warning he was hesitant.
Long story short I started back up on a very low dose steroid and allopurinol. Essentially take 5mg every day for a 1-2 months because I’m going to have flares even while on allopurinol.
Sorry to hear about such a struggle and the pain you're having with these flares. I've talked to several family members who all have had long term gout problems and they all said that they were instructed to stay on the Allopurinol even after having the early flareups. All of these family members have been flareup free for a few years now. And, about the Uloric: two of my family members had to get off it because of the black box warnings. Hoping you get some peace and relief with your current medication regime.
