hello , can anyone explain :

1)high protein creat rato despite normal creat ratio??

2) oedema despite low normal albumin and with dehydration!!!

Hi Samaka,

I can't explain your first question, but I can help out with the second question. I have this same problem, actually! How it was explained to me, was that it was an absorption and excretion issue. Since kidney disease can impair the kidney's ability to remove excess fluids, it has nowhere else to go except into your tissues until you remove it either via pee or dialysis if you are doing dialysis. The fluid in your tissues is considered waste, so your body will tell you that it needs more water, even though your foot is swollen. Are you pre- dialysis or currently undergoing treatments? If you're pre-dialysis and this is a frequent problem, talking to your specialist about potential diuretics to make you pee more often can help. Getting up and exercising between elevating the legs is also something that I've been encouraged to do since it helps to keep everything moving. Believe it or not, keeping a water bottle with you at all times is also something that can help. Your body can't get rid of waste if it doesn't have enough water for cellular respiration!

the first question is labs done ,one of them called Protein creatinine ratio (mine is high) and the other one called albumin creat ratio (mine is normal !!)

i’m not predialysis but in severe dehydration and diagnosed Nephogenic Diabetes insipitus but first time to be swollen like this !!!

will try to increase water intake 3 litres daily?? , thanks a lot for advice

Please don't increase your fluid until you've spoken to a medical doctor as the last thing you need is the fluid to building on your lungs as this could cause a strain on your heart. I'm on 550mls fluid restriction everyone is very different please call someone and explain your situation.

How much are you currently intaking? Usually with edema you would have some sort of liquid restriction. Like I had said previously, talking to a specialist about options like having a diuretic or if you need to increase water consumption would be something that needs to be done first!

intake is according to urine output (fluid balance ) 75% from the output

I know this is going to sound weird and I really don't mean to offend but you can get swelling when you are malnourished.

How is your eating as I know with renal disease many of us struggle with low appetite a d then with restriction it can be hard to get the nourishment you need.

What is your HB level ? commonly when your kidneys aren't making enough erythopoietin then your hb levels are low as not enough bone marrow to produce them. It's sorted with epo injections and really does help.

I'm just recovery from 10kg of fluid overload but this was from all my levels being all over the place and renal function falling to 4.5% and need for dialysis. This happened to me 2.5 years ago and bounced back with the help of epo. So as you can see it can be a number of things.

Try and stay as active as you can as I find it really does help. If I walk for 30mins idea again it helps keep on top of the fluid.

I would call your renal doctor to see them as it needs checking as although it's in your ankles it could be in your lungs too. It's a sign something isn't right so worth a call that's what they are their for.

Again I hope I've not offended that was definitely not my aim or to scare you.

my nutrition is very bad really , i can’t eat well , u mean proteins or albumin deficiency?? or generally?

My Hb is decreasing a lot , i just had blood transfusion, but never thought its from kidney ! i though just bleeding

my renal functions are normal because my disease only affects salts and makes me seevrly dehydrated

is it really that if i have muscle atrophy and i cant wat well , the creat will be always normal despite renal impairments??

I mean in generally I hadn't realised that malnourishment could cause swelling until my consultant mentioned it to me as I was suffering from sickness a lot. But when you think of all those starving children in the third world all have swollen tummies and this is from malnourishment so makes sense.

Low HB can cause the issue with erythopoietin. Google it and then maybe ask your doctor about it.
