I have never seen evidence of any herbal or other supplement that was effective for cytoreduction, though there are some that can be helpful in other ways. One example is curcumin, which i have found to be an effective anti-inflammatory agent. It works better than NSAIDs. Theoretically, it might help with inflammation-driven thrombocytosis but I have never seen a difference having taken curcumin for many years. There is a large reduction in osteoarthritis symptoms, however.
Things I have read about include quinine, red wine, cranberries, tahini, pomegranate, raw garlic, walnut, dark chocolate, oregano, cloves, cucumin. I include them in my diet and many of these have anti inflammatory properties I believe, but curcumin is the one most written about and I do take this. However, I have seen no reliable difference to the platelets. Platelets are always something I am chasing, but thankfully the hydroxy makes a difference.
Shortly after I was diagnosed with JAK2 ET (Jan 2024), I consulted with a naturopath about how to reduce my platelet counts. My platelets had slowly inched up from the low 400's to almost 570 across a few years. She recommended theracurmin and said she thought we could bring my platelet counts down a bit. I was a little skeptical, but I was also desperate to push the hydroxyurea decision out into my future.
The product she recommended was Integrative Therapeutics Theracurmin HP (bioavailable curcumin). I started taking 4 capsules per day, two morning two night (total 1,200 mg). This might be a coincidence, but my platelets gradually dropped from 570 down to 471 by late September. At my most recent visit to hemo, my platelets were still at 490.
As always, let check with your physician before taking something new. Curcumin can have some gastrointestinal side effects (and can cause liver damage, I think).
My platelets used to float around the 500 plus area, now they have been consistently in the very low 400 zone for 12 month's.I eat quite healthily and exercise daily.
Take 75mg asprin daily.
I added a own formulated juice/smoothie to my routine that consists of strawberries, frozen broccoli, kiwi, small orange, 4-5 walnuts & 3 small beetroots .
Maybe it just works for me...seems to be doing something.
Jak2 positive ET since 2019. Ive never taken cytoreduction meds. Spleen is good. I’ve had success using quinine (tonic water) & dark chocolate to lower my platelets - consistency is key to address the symptom of high platelets, but doesn’t address disease progression. I’m on a Gluten free diet and I take Curcumin, Vitamins C,D, Omega 3, & Cytokine Suppress supplements. The last two years I’ve taken plaquenil for an 10+ year autoimmune condition. Recently, I was diagnosed with PV with no other symptoms except elevated RBC. Because my allele burden is 28%, I will begin Besremi treatment this summer.
Diet & exercise (yoga) help tremendously.

I tried for 2 years to avoid medication. At first platelets decreased but then steadily went up .I've tried so many herbs , homeopathy, ayvedic approach, ( all under advice from professionals)
When platelets rose to just over 1000 I decided risk of stroke or heart attack was to great and now take hydroxicarbamide every other day.
As well as CBD oil , medicinal mushrooms and some vitamins and minerals
can afford you see a functional practitioner who can tailor a plan suited to you as well as offer emotional support ?

I would love to see any evidence of any natural products lowering any counts with MPN , and believe me I have looked and tried many. Everyone to their own but I wouldn’t waste the money or time or both , I did that too. 😀
