Am I pipe dreaming or can we get a central point for hospitals doctors dentists. Dentists because I was on tablets for 11 years and they dissolved some of my gum. The dentist didn't know even though I had told them. I moved so my fault. Anyway they should all be connected. It's one body lots of Dr but no communication. Any suggestions.

Don't get me started on that because I was on Hu and have et jak2+ my consultant referred me to the hospital dental unit years ago and I broke my front tooth a few months ago so phoned for an appointment only to be told I had been discharged from the hospital dentist see trying to find a NHS dentist thankfully after going all over Edinburgh I have found one I asked my consultant to refer me back to the dental unit she said no but don't know how will get on without safety net off my notes at dentist hand and trying to explain to the new dentist my conditionStay safe

Hope you get sorted out soon! Wish someone could start a unit to join everything 😞 together. I have ET jak2 dr surgery lost my notes and accused me of not handing them in. My carer my daughter was with me. Someone in authority must be able to sort the whole thing out as someone is going to die .

We have to be our own best advocates and case managers. It is never safe to assume that information is flowing though any system. This requires a high level of organization on the patient's part. It really helps when providers have patient portals that include all of your labs and other medical records. This allows you to move the records between providers yourself. It is necessary to rely on hard copies sometimes, but this can be done too.

I go to all appointments with a written agenda that includes updates to all medical conditions, a complete list of all of my medications, and a list of all medical conditions that I have. I frequently have to correct something in my chart. The effort is worth it since it prevents mistakes and improves quality of care.
