Ruxolitinib has been in clinical trials for ET, but does not have a label indication for ET. It has been used off-label for ET. Due to the considerable expense, accessing ruxolitinib for off-label use can be a challenge. Note that technically, hydroxycarbamide use for ET is also off-label, though it is in common use. However, hydroxycarbamide if very inexpensive. To compare the retail costs in the USA, HU = $25/60 caps. RUX = $14,308/60 caps.
So to answer your question, ruxolitinib is used off-label for ET, but it is not common due to the expense. The two recognized first-line treatment option for ET are hydroxycarbamide and Pegasys (which is also more expensive ($4,300/month). Both of these options are off-label for ET, but are in common use.
