Yes - this can be an ET symptom. Erythromelalgia can occur in the toes or the fingers.

Note that we can get this kind of pain in the extremities with MPNs without it being ever enough to be called erythromelalgia. That happened to me. The MPN Specialist just called it "PV Toes."
Since this is a new symptom you should definitely let your care team know ASAP. They can best advise you how to treat the issue. Some find that increasing the aspirin to 2x/day helps; however, that is not something you should do without consult with your care team.

I have red painful fingers and Prof Harrison’s team doubled my clopidogrel which helped a lot.
This is exactly what I experienced 5 months before my diagnosis of E.T. My fingers were warm to the touch, swollen, red and painful. I went to 5 doctors and two natural paths, until finally someone thought to send me to a hematologist. Perhaps you are in the early stages, but think about watching your platelet level closely.
