Hi all, I wrote back a month ago worried about my lowering platelets and the doctor taking me off Besremi. My last injection was early November 500 mcg. My platelets got down to 53! They are coming up, now at 87, 2 weeks after the 53 reading. The Besremi is still having a lowering effect on my white and red cells, both now in normal range. Unfortunately, my liver enzymes are also affected: ALT 53, AST 52, Alkaline Phosphatase normal but at the high end. I had a BMB yesterday and will get results in Jan. Although nervous to hear about the biopsy, I am glad my platelets are increasing. Take care friends.

It is not surprising that your platelet levels are that reduced at the 500mcg dose. Many of us are being maintained at target at Besremi doses considerable lower than that. Elevation in liver enzymes is pretty common with Besremi. My docs are not concerned with it until they get upwards of 4x-5x max base level. I am on 150mcg Besremi. My last CMP showed ALT at 124 and AST at 87. Alk Phos was 167. My care team is not concerned with liver numbers at this level. No change recommended.
Hope you get good news from the BMB. I just got good news from the most recent check on JAK2 Quantitative Analysis. My JAK2 allele burden has been reduced from 38% to 9%. The Besremi is most certainly doing something besides maintaining my hematologic response.

500 mcg is a very high dose. I just increased to 200 mcg for my last 2 shots. I have a very high allele burden: 89.9% !! I’m hoping after 6 months on Besremi to get another test & hopefully it’s going down.
You've been off the Bes for about 1.5 months, is that right? It's good that your red and white cells are holding in range with that much time off. It is known that some can discontinue IFN and hold blood counts in range, but that is normally after much longer with the therapy.
Is there a date you will restart the treatment? If the blood counts hold maybe there is no rush, and the plt can continue to increase. What is Dr's thoughts?
