I am assuming that the injection you are talking about is Pegasys, one of the two first-line treatments for ET. We are all different in how we respond to these meds. Some people do indeed find significant relief from the cytoreductive medications. Others have trouble tolerating the meds.

I have PV, with both erythrocytosis and thrombocytosis. It used to be ET, but it progressed. I could not tolerate hydroxyurea. I have done much better on the PEGylated interferons (Pegasys/Besremi). I do in fact feel better now and the side effects have been minimal.

Given what you describe, the doctor's recommendation for cytoreduction certainly makes sense. Of the two first-line options (Pegasys, hydroxycarbamide), Pegasys is usually recommended for people under age 60 due to the long-term risks associated with hydroxycarbamide.

It may help to learn a bit more about treating ET. Here are two articles that may help.

Suggest that you learn a bit more about Pegasys and your other treatment options then go back to your care team with your questions and concerns. For what it is worth, I have been very pleased with my response to the interferons. My only regret is that I waited as long as i did to try this course of treatment. You will have to decide for yourself what will be best for you.

All the best.

Since May 2021. The plan is to continue indefinitely, possibly for the rest of my life. Certainly far longer than 2 years. There is not 2 year limit on the IFNs that I have ever heard o

I have been taking Pegasys for a year and a half and am very pleased with it. As your figures are controlled you should feel better. The nurse should show you how to take the injection which is easy really. I would recommend starting on a low dose of 45mcg to let your body get used to it, and increasing as necessary. Drink loads of water. We are all here to help with any questions you might have. Good luck
