How does Besremi work?

I read all the material on their website, but I'm still not sure how Besremi impacts the bone marrow in PV. Normally interferon suppress your immune system, right? Does it reduce the allele burden?

You have a good question about bone marrow and other effects, because even the experts don't know at a deep level. It does cause the mutant marrow cells to be more exposed to destruction in the body as I understand it. Mostly they know from observations that it works, but less about how.

It does usually, but not always, reduce allele, sometimes by a lot. Some members here have posted these benefits for the older PEG.
