I don't know what is grade 4 phimosis (severe?), but if just the tip of foreskin is tight it might be possible to snip off that part. I've heard there is a risk of secondary phimosis on that time when the scar is first 3-4 months like tight rubber band.

And you might be scared of pain and tightness, but proper wound care is still then necessary to avoid infection under foreskin. Have you already talked with you doctor about your condition?

What is good Alisklamp or zsr? What's the difference?

Hi, you need to speak to a urologist who has good experience of circumcision with grade 4 phimosis - which I believe means very minimal retraction.

As I understand matters, circumcision in the case of phimosis of this degree would typically require an initial dorsal cut along the length of the foreskin as far as the corona which would not lend itself to a partial/high and loose style circumcision.

If you wish to retain your foreskin, it would make more sense in the first case to explore stretching exercises, in conjunction with topical corticosteroids. Some men report having achieved fantastic results with time and patience. You could also explore less invasive surgical options such as preputioplasty or a simple dorsal split.

I wish you all the best.

Very good advice from Nicktremayne. Also you don't have to consent to anything you are not at peace with. If you want to keep your foreskin you should be given every possible opportunity and support to do so by your GP and urologist. Don't be afraid to make your preferences and anxieties known to the doctors advising you. Don't be afraid to seek second opinions.Best of luck
