Caregiver checkup

I ran across this Caregiver checkup on a Palliative Care site and thought I'd share it with the group. Sometimes I use tools like this as a scheduled self check up once a month as a way of checking in on how I'm doing, and if I'm coping worse, I take steps.

I support you in getting more help. Sometimes we are so busy caretaking that we lose track of the impact on us.
