I ran across this Caregiver checkup on a Palliative Care site and thought I'd share it with the group. Sometimes I use tools like this as a scheduled self check up once a month as a way of checking in on how I'm doing, and if I'm coping worse, I take steps.

See what you think about this tool and if it's helpful to you. Today I scored a 4 (scoring instructions are on the second page, and is a little tricky, but you can do it!).
