This is something that people do not have very often a dislocated oesophagus?

I was born with a deformed chest 64 years ago, but due to epilepsy have broken practically all my ribs at one time, my oesophagus dislocates especially while trying to get to bed, my breathing is somewhat restricted. I cannot sleep on my back and due to skull injury not on my left hand side. So I ALWAYS sleep on my right hand side, recently this dislocation has been getting worse, always on the same place on my sternum, affecting my heart beat, I have had two ECG's which have proved my heart beat is ok and regular when lying on my back. My local hospital especially as I have been diagnosed with early dementia regards me as an "idiot" treating me thus, even this could be explained as recent changes in epilepsy medication. It always happens when the rib [mid sternum about third one up] horizontally casually clicks out of place when trying to sleep, my breathing is severely restricted, and losing sensation in my hands and legs. I have to get up for a couple of hours to regain some sort of normality. I am retired and live on my own. Who would I go to see about this, it has been worse since just before Xmas, I have diabetes 2 since March 2020, and because of this pandemic severely restricted in seeing any medical help face to face?
