When trying to have a conversation I am often told I am mumbling incoherently and then I have to repeat my self slowly

I find when I've been talking a while, I start to lose my voice as well as start to have problems finding the right words. It happens on the phone as well as face to face with people, I have to take deep breaths and pause before I can continue. It happens with family too and it's worse as I get more tired.
I have exactly this, it can get really embarrassing! The more wound up I get the more I can't find the words I'm looking for until I just loose the whole conversation. The only thing I can do then is admit I've forgotten as I can't carry it on! People laugh, don't think I'm serious! Then two minutes later I can pick it up again. I went to my gp once as my mumbling was getting worse, she told me to 'try not to mumble' I could have punched her. I still could. Grrr!
Further to Meningitis & a Stroke, I suffered Nerve Damage, so I have slurred speech, when I telephone people who don't know me, they wrongly assume I am Drunk & put the phone down!! (I can't even swallow!!)
It made me so angry, I now I start by saying I am Disabled & have slurred speech!
Wish I knew the answer. Why can you get the words but, other people can't.
Mind you as an addition to my question not only am I incoherent but when I do get the words out it sometimes comes out as something I would never say in a million years
Hi Dillm, stopping me talking is the problem -I'm as bad when writing - as you know and can see. I've not been accused of mumbling - but have always spoken slowly I have a need to explain every last deatail now but don't know why now I keep losing track of what I was going to say .and a load of nonsense comes out - like I'm discussing several things all at once! So yes a communication problem I guess.Was nevergood but definitely worse. Shirley.
