My recent MRI presented normal, but DRE indicated a lump or hardened area on the prostate. PSA is almost undetectable. Otherwise healthy. Any ideas on what could cause abnormally low PSA count other than an aggressive form of cancer? Biopsy won't be for another 3-4 weeks.

Can you give us some info about your history... what your PSA has been, blood in urine or ejaculate, difficulty urinating, age??? In the study below, 44% of men with an abnormal DRE had prostate cancer....see below:

I am 64 years old. Back in 2010 my psa was at 1.27. By 2017, it had dropped to 0.08, and most recently (January 2021) it's down to 0.06. I'm told those are numbers of someone without a prostate. My prostate measures 34cc from the last MRI. MRI showed nothing that would indicate a lesion, though there is scar tissue from a bout of prostatitis. There has been no blood in urine or ejaculate, but urine flow has slowed down, which one would expect with an enlarging prostate.

Since you had an abnormal DRE, a biopsy is indicated and has a good chance of detecting possible prostate cancer--MRI guided biopsy.
