How do I get a copy of my assessment from rehab

Hope every one is warm in well

I attended the rehab centre in Irvine Ayrshire, , how can I get copy of assessment to prove to tri I can't walk?? Thanks

If you just ring the centre and explain what you need, I'm sure they will oblige.

You are entitled to a copy of any notes concerning you, and they should have no reason to refuse them.

Your GP practice will almost certainly have a copy as well - it might be quicker to ask them first
