Hi ladies,

Does anyone know when or where I am supposed to get my MATB1 form from. I’m currently 29 weeks and work are asking for this repeatedly. Im not seeing my midwife until November 6th and I’ll be 34/35 weeks by then?

I’m sure they normally give you it at the midwife appointment after your 20 week scan. Mind you, the last 2 pregnant ladies I manage at work had to ask for them as their midwife forgot and we need them before 30 weeks to organise Mat leave. You may need to call your midwife and they’ll arrange for a it to be done x

I got mine at my midwife appointment when I was about 22 weeks pregnant I’m sure, I needed to send it to my employer 15 weeks before baby’s due date (I think... my memory is terrible 🙈). Could you maybe phone midwife and ask if you could pick it up, it took minutes for my midwife to fill it out. Hope you get it sorted. X

Midwife at 21/22 weeks I think no one else can give you it a

So ask midwife xx

You can get it from your midwife or GP from 20 weeks onwards. It needs to be signed by either.

Hiya I had to go to my gp. I just called up and asked for one and had to collect it. My midwife didn't issue me with one. X

I got mine at my 25 week appointment with the midwife x

Midwife should have gave to you at 20 weeks :) maybe give her a call x

You definitely should of had it at 20 weeks as that’s when employers usually ask for it ! They must of forgot ! Give them a call and you should be able to collect it from drs x

Your midwife should of given you one at or around 20 weeks. X

Your midwife should have given it to you before now. Give her a ring and ask for it.

Your midwife she should have issued upon request afta 20wks. Keep trying your midwife say you need it for wrk
