Feeling very frustrated

Fibroscan last month confirms no fibrosis. gastroenterologist says given that I’m 63 I’m unlikely to get it now. to come back in 12 months after another blood test.

In the mean time my dr asked to have a blood test in 2 months which was this week. I’m seeing her on Monday. Just wondering why my ALT and AST scored would have risen so much in this last blood test

Scores are up from 38 to73 and 28 to 37. GGT 29. What could have made the ALT go up. Do they fluctuate

I’ve been strictly eating all the recommended foods since being diagnosed with fatty liver 4 months ago and increased my exercise. I’m not much overweight maybe 6kl and generally eat very healthy. I’m very disappointed. These levels are the highest that they’ve been in 5 tests over the last 1 1/2 years.

Those are an indication that liver cells are being damaged but don't tell you why. A viral infection or chemical could be involved. Anything that increases inflammation will do it. Could be an auto immune response. A skilled doctor will help you figure it out.

Thanks Nash 2. Any advice on how to find a skilled dr? Do you mean dr or did you mean gastroenterologist

My advice is to find a hepatologist, even most gastro docs are not well trained in liver disease

My gastroenterologist also specialises in heptology and after my fibroscan came back OK he said he didn't want to see me for a year. The only advice he gave me was lose weight and exercise. I weight 75kl. Now I'm not sure if I'm making too much of these latest lft results going up. But it seems to me a lot of people on this site were told they just have fatty liver and aren't told they have to manage it and then sometime later they have fibrosis.

Liver function tests show that liver cells are dying. The higher the value the more cells are involved. A lot of things can cause them to be high so it is fair to want to know why and it isn’t necessarily NASH

Well just a few examples alcohol, toxins, infections, autoimmune disease, cirrhosis, sleep apnia. The liver is the most complex organ so lots of things interact with it. Too many to list but fortunately it is also very tough

Sleep Apnea causes liver issues?

Yes, it contributes. It causes reduced oxygen levels which makes it harder for the liver to battle inflammation. It is a comorbidity not a direct cause by itself

What about choline deficiency? Have you any thoughts on that? I’ve been reading some really interesting articles about its connection to NAFLD.

Choline is critical to health and can be a problem with liver disease. It occurs in many foods and a well rounded diet can provide it. The liver produces choline so it is harder to maintain blood levels if the liver is sick. The need for dietary sources is one reason we don't suggest a fully vegan approach because meats are a good source. Eggs are quite high in choline. Here is some reading if that is of interest.

Feeling a bit down. Just had another blood test and now I've had a call saying the dr wants to see me about my blood test results. I don't want to go. I know she's going to tell me the levels have gone up again. I know I need to pick myself up but right this moment I'm feeling defeated.

Too many days feel that way with this disease but you do have a lot of companions on the journey and we wish you well

GGT is within the normal range. Fibroscan indicates you have no fibrosis. what are you worried about??

Guess I’m not sure if I should be worried or not. So are you saying it’s nothing to worry about if you have fatty liver disease and your ALT and AST levels increase.

I'm in the same boat. Can't say I don't worry, but what I can say is that alt and ast are going to go back and forth with the disease. I try to keep an eye on how elevated they become. Before my diet when I was 330lbs my alt and ast were 50/77, the lowest they have been since my diagnosis in 2017. Then I lost 70 lbs, then my alt went up to 125, but ast went to 60. One doctor said it was because of the diet other dr said it was because fatty liver. Who knows. Also, I plug my numbers in to the calculator to see if the ratio is concistent with cirrhosis. Also, I watch out to see if ast is highter than alt.

Thanks JJ. Sounds like you are working hard on your recovery.

I’m just new to this and trying to understanding what’s going on. I’ve been doing the usual research but the rise in the levels in the recent blood test were a bit of a shock. I think I naively thought I’d do the right thing for a few months and I’d be cured.

I am too. I think about Liver disease 24/7. I research alot and still know nothing. Not sure if I am getting better or worse

Good luck then. I think I'm going to see another one and get a second opinion because mine told me to lose weight and exercise and come back in 12 months.
