4 year old can only poo in certain position

My 4 year old has always had trouble with stool being too soft and sticky, she was recently hospitalised as she got completely blocked up for 2 weeks and needed intervention, she is on movicol and is able to pass but she still can only manage it if she is wearing a nappy with her feet under her bottom, which is obviously not going to work on the toilet or potty, it seems like there is a positional thing going on? We know that squatting is best but she needs her feet right under her, has anyone else experienced this?

Possibly still witholding, you need to add a stimulant to the regime, also maybe invest in a squatty potty too.
