3 year old chronic constipation

My child goes 7-8 days before going. She was taking 15 ml of lactulose 2 times a day. We also tried miralax for a long time before that. sShe also takes exlax of varying amounts and done clear liquid only with high doses of miralax clean outs once a month for 3 days. Nothing seems to give any real results. I make sure she drinks a fair amount of water everyday. She sits on the potty after every meal plus any other time. She was potty trained but is now back in a diaper even though she is nearly 4 to try and allow her to not be scared. We have been dealing with this since she was 2 1/2. I’ve also tried lots of prunes and fruit smoothies with high fiber fruits. We have used suppositories but they only produce a result around day 6 or 7 when she is already in a lot of pain. I would love some recommendations of how to help my child stop holding it.

Add a stimulant to the regime and change yo a different osmotic laxative, lactulose isn't that effective unfortunately.
