I'm not entirely sure but I guess what you need to do is go to your GP with any letters and documentation you have from Italy. Can you translate them yourself ? I would translate them and fix the translations to the originals,if you are an English native speaker and your Italian is good this should be fine, if you are a native Italain speaker you might just want an English native speaker to read them through and make sure they are okay. The letters will have contact numbers/e mails for hospitals/clinics/specialists in Italy. So your GP can conact them if they need more info. Pretty much everyone who is well qualified in Italy speaks pretty good English so they can contact them.
If you don't have any letters etc with your diagnosis and listing your medicines etc you will nedd to contact the places that have treated you and ask them to write everything down to give to your GP, then you can translate them or get them translated. Then the GP can add all this info to your medical record and get you started on any prescriptions you need.
Looking at your two pieces of writing here I'm guessing you are an Italian native speaker. These diagnosis are definitely recognised over here but the treatment you get prescribed might be different. Professional translation of documents is really expensive if you pay per character. Look through your documents and let me know how many pages of info you need to translate. If it's not too much I might be able to help.

Hi! I was diagnosed abroad and translated any medical letters myself (MRI report/ultrasound/surgery) stating the diagnosis, and sent it to my GP! Hope this works for you x
