You can request them. There will be a fee.

Contact PALs at the hospital you want the notes from, they will then tell you the procedure to get them. I don't think they are allowed to charge for this anymore other than postage charge (but even this not all will charge for it)

My GP printed off mine. I lived abroad for many years, so got into the habit of requesting a copy for my records.

Your GP should be sent your hospital notes, so you can request them from there. If not then call up your hospital, as it's all electronic now they can search and pull your records from however long ago and doesn't matter which hospital you were at, they may need you to go in and collect them. If you're really stuck your GP should definitely be able to request them on your behalf, but they should see them anyway to know what's going on especially if they've started the referral.

hi im glad i came across this post as this is something i must do as my gp and my pops gp ( separate surgeries ) have told us both on separate occasions that somehow 4 years worth of notes has disappeared mines not so much a worry too much as mine was about 8 years ago but my pops is from 5 years ago when he was in hospital for a few months due to seizures giving him brain damage so this is important information that is somehow no longer on his file and we need to find them so this was handy signing in to find this post, thank you and i hope you get yr notes found good luck xx

They can't charge you for them anymore. New GDRP laws prevent them charging.

I know for NHS grampian I just downloaded the form off the trusts website and sent it to them. I had a copy of all my hospital notes in under a week, and all my radiography studies on disk.

You will need to write to your GP practice manager and request your GP notes seperately (they are not on the same system and are not stored in the same place so you need to do the leg work sadly), but again under the new GDRP laws they have to give you them and they can not charge for them.
