Can anyone help in regards to moving county's and changing hospitals during the middle of the process of seeing a gyne?

I'm currently registered in one county and in the process of getting an appointment for the gyne which has taken me 3 and a half months.

I have just moved into another county and was curious if I need to start the process fresh with my new doctor or can I request them to transfer me over to another hospital into a different county??

anybody had this experience?
