HOMA-IR: Homeostatic Model Assessment of Insulin Resistance. It marks both the presence and extent of insulin resistance that you might currently express. It is a terrific way to reveal the dynamic between your baseline (fasting) blood sugar and the response hormone insulin.
HOMA-IR = (Glucose x Insulin) / 405 (Glucose in mass units mg/dL)
HOMA-IR = (Glucose x insulin) /22.5 (Glucose in molar unit mmol/L)
Normal Home IR value of healthy human ranges from 0.5 to 1.4 ...
Less than 1.0 means you are insulin sensitive which is optimal.

One does not need to go any hospital. It is available at most of the test labs. One can order along with FBG test. Postprandial glucose and insulin levels are also checked some time. Insulin test alone has limited use.
Please note that IR score is still not an universal test marker because enough data has not been collected on larger populations. There are various factors which may affect the normal levels., ethnicity being one. That is the reason most of the labs provide a very wide reference range for this test. Having said that this test seems to be quite useful in understanding the underlying problem even when blood sugar is not high. One can take preventive actions against future T2D.
I got done fasting insulin more than three four times. Everytime it showed quite different values ranging from 9 to 23.....which is normal. Fasting insulin actually depends on what you ate last night...etc...so the different foods may give different fasting insulin values...this is what I think. Moreover frequency of eating also might have some effect on fasting insulin next day.
I did it long back...IR by above formulas normally came between 2.5 to 3 because I am diabetic. My fasting sugar level initially when I got diabetic was around 100 every time...but after 5 years now, it is been on higher side...like 120-125 which is again acceptable for a diabetic. I go more for HbA1c results which give you rather a controlled blood sugar level of average of last 2-3 months. Mine HbA1c ranges from 5.8 to 6.9 with couple of times above 7 like 7.4 or so. I just try to keep HbA1c results less than 7 because I do not wish to follow strict diets as I am strictly vegetarian...can't replace carbs with non-veg protein like meat or even eggs.
I take Metformin 1000 mg tablets twice a day morning before breakfast, before dinner & Tenegliptin 20 mg before lunch. I take Ayurvedic medicines also twice a day. No diet. I eat everything I used to before
Thanks Ashka,But if your fasting glucose was around 100, what made you think you were diabetic? my fasting was 291, 2 hr pp 541, and HbA1c 12.7% when I was first diagnosed 5 years ago
Metformin gave me diarrhea and Glimepiride made me lethargic, so I was immediately put on insulin, 3 months after that my HbA1c was 7% fasting was always below 120 and 2 hr pp were also within normal range. I test my HbA1c every 3 or 6 months after that and the numbers are always between 6-7 never passed seven, 6.7 6.5 6.3 6.0 6.4 6.2 6.8, etc
I got my Homa IR and Homa B tested 4 years ago, Homa IR was 2.1 and Homa B was 53.4 (normal range 70-150%) when those tests were done my fasting glucose was 113 and HbA1c 6.5%
My fasting glucose was 135 when first diagnosed & HbA1c was more than 9. After that I started taking medicine. After taking medicines also, fasting glucose was initially around 100 but then with time disease develops so fasting glucose is more now around d 125
