At 46 FBS without symptom seems impossible. May be the meter is incorrect ! You may like to recheck in a Lab.
Also is he taking a Beta Blocker - a BP lowering medicine? Many times Beta blockers known to mask the symptoms of Hypoglycemia leading to death.
How Meter can be checked...yes he is taking beta blocker...but how it can block symptoms
Beta blocker blocks the pathway which signals the hypoglycemic symptoms to the brain.
Hence Beta blockers should not be administered with glucose lowering medicines.
There are many effective BP lowering medicines ACE/ARB/ CCB and many diuretics are available as options for diabetic.
